Dog Registration

All dogs over five months old must be registered. A current rabies vaccination certificate is required before a license can be issued.  After you submit this form, you will be prompted to pay the registration fee (there is a card processing fee).  Please call the office with questions, 920-989-1589.

Fixed dog- $10 each

Unaltered dog- $15 each

Late fee- $5 (per dog) after March 31.
